I currently have a 3 speed manual with a granny low in my '86 ck 1500. I don't do too much highway driving, but as it is I am running 3,000 rpm @ 70mph. I'd like to be able cruise at 85 and don't necessarily need the extra power in low gears. Any suggestions?

How involved would that be?

Buying a new gear set for around 200-250, and having a shop with the required tools set it up for you for around 2-3 hours labor. There are several critical measurements that need to be done precicely to ensure the new gears live a long and happy life...LOL

700r4. That would be mu choice. The cost might be a little higher than a ring and pinion swap but well worth it in the end. Unless you just wanna bang the gears yourself then you need a 5 speed but the later versions used in the newer trucks are junk.
